Set Up Your Amaze Account in 3 Easy Steps
Step 1: Download the Amaze app
Search “Amaze Health” in your app store and install the app for the best experience.
Step 2: Enter your username
To find your username, search your email inbox for “Amaze Health” to find a recent communication that shares your username. If you’re still unable to find your username, follow this link to retrieve it.
Step 3: Set your password and log in
Open the app, click forgot password, and follow the prompts to set your password. You’ll receive a secure setup link by text message or email, based on the contact information we have on file.
Once you’re logged in, tap “Send Us a Message” or “Call Amaze” to connect with our medical, advocacy, or billing teams.
Use the three-bar menu in the top right corner to edit your profile and add your family members. Amaze covers your immediate family members, including your spouse/partner and children up to age 26 years old.
